Sickness Certificate (Fit Note)
If you are off work because of illness for seven calendar days or fewer, including weekends and bank holidays, you do not normally need a fit note. You can self-certify your absence using an SC2 form, which is available from your employer or through the gov.uk website.
If your employer requires medical evidence during the first seven days, we may be able to provide a private certificate. There is a charge of £18 for this service.
How to request a fit note
Please submit all first and repeat fit-note requests through our Accurx online consultation service. Do not use this service for advance requests before a fit note is clinically required.
Fit-note requests are requests for medical assessment and will therefore be clinically triaged in the same way as requests for same-day appointments. The clinical team will review the information provided and decide whether a fit note can be issued or whether a telephone or face-to-face consultation is required.
Submitting a request does not guarantee that a fit note will be issued or that you will receive a same-day appointment.
